insurance billing for Pulmonologist?

I work for a new billing company that has just taken on insurance billing for a pulmonologist. I have done billing for ambulance services, MRI facilities and Radiologist but not for this type of doc. Does anyone know what type of info if any needs to be sent with the insurance claim??? We are based in Kansas.

Other than the HCFA, I'd assume any referrals that are required by the plan.
