you can make one on your own on MS excel, it will not be urine intake and output, it will be simple fluid intake and output and then you can make categories of different fluids for intake, the output is easy to measure in hospitals since they have callibrated jugs for urine and bins for stool, but stool measurement is difficult at home, good luck
