My first thought is that this sounds like a recurrent virus or maybe even an allergy. If the baby has been having other issues which require hospitalization, maybe her immune system has been compromised? My first suggestion is to start a diary for her. Note down all events in her life from going on a car ride to the grocery store to what she eats (as well as when and how much). Include her temperature on a daily basis.
When you note her temp going up, see if you can detect a common thread. See if maybe her temp goes up every time you take her to a home with a pet, or if it goes up when she eats applesauce. This will give you insight as to weather or not there is a commonality that you can eliminate from her regime. I hope this aids you.

A fever can actuall by a good thing. It's a red flag to let parents know the body is trying to fight off some type of infection. However, that's not always the case. My cousins baby showed the same signs as your baby, and her baby was teething real early. At three months old he had two teeth. I couldn't believe it.

Minor infections. Sometimes vaccines can cause them too, because the body is assimilating the new germs into the immune system's code.

Contact your doctor. If the doctor doesn't address this to suit you, then get a new doctor. Could be any number of things.
