yes, do. you didn't say which country you are from, but if the date of birth is clearly wrong, you have to send it back for correction. If you don't, and you use it, it contravenes laws.
One thing... most passports have the following format: dd/mm/yyyy
example 16 09 1895, or 27 Feb 1763, but in America, it is mm/dd/yyyy for example 09 16 1895
check your passport again to see which format it is.
clarify with the relevant passport office anyway.

Yes, you need to go back to the passport office and get your DOB corrected because DOB is the most important part of our Identification. If you apply for any type of Visa, the 1st thing the immigration checks is the validity of your DOB, by comparing all your records like birth certificate, school leaving certificate/ driver's license [if you have any]. The passport office will probably mention the change manually inside on one the pages of the passport. Immigration is a crazy business, so get it corrected ASAP.

I would get it changed. In today's world of International travel, it is always best to have your documents in order. If something were to happen to your passport while overseas, you would be required to travel to the nearest consulate/embassy to have another pass reissued. That process in itself can be very frustrating but if your initial passport was wrong to begin with, the process could be even more grueling and the country you chose to visit for vacation may turn into a much longer stay than intended. Even more of a problem depending on the visa requirements for the host nation.

Go to the passport office and fix the error. (Inside post offices in the US) You can also fill out a form and send it in with the incorrect passport and they'll send you a new one.

This is part of the serial number of your passport makeup. I wouldn't worry about it. If you are worried call the passport office and describe this and read the person the number and they could maybe check it right there. Hope this helps.
