well if both your borther and sister have boys and girls then you can't. unless when you talk to people you say "you know my brother's daughter, my niece mia..." and so on and so on. that is the only way people will know if you are talking about your brothers or sisters kids.

It's the same word in English.
If I wanted to be specific, I personally would use the nephew's name (I only have one niece) and/or say "my sister's son" or "my nephew on my husband's side". I only have sisters and my husband has one brother (father of the niece).

i dont think this is commonly differentiated in english. for example, in urdu, u use different words for your brothers' and sisters' sons and daughters.
i suppose in english u can only use their actual names, or say "my sister's daughter" or "my brother's daughter" etc.
