You can go to Microsoft Word help on the web and download a variety of templates and then you can choose which one would be more appropriate for your circumstance.

Not sure I understand your question. You have to be more specific, like what kind of job are you applying for? If it is a professional occupation you want to include a cover sheet and reference page. If it is not "professional" just keep it simple. If you have Windows, you should have a resume maker in your Microsoft Word.

Depends entirely on the job. For most it's an eye catching (basically true) (but embellished) ad to get you an interview.
If you've been a goody two shoes it can be entirely honest and concise!
