Sir, I'd like to ask for your daughter's hand in marriage.
(Be prepared with a thorough explanation of how you are going to care for his precious little girl.)

You go to him and ask to talk privately. Then you start by telling him how much you love his daughter and that she makes you whole and you want to marry her, but you want his consent before you pop the question.

If she has said yes, you call him and ask him if you could speak with him. You dress nicely and meet with him. You say, "Mr Blahblah, I asked you to meet with me because I would like to ask your permission to marry your daughter." Then you blush and wait for his response. Congratulations on being a traditionalist! It will go a long way in helping you to become welcome in her family.

As respectfully as possible.
It helps if you've already met him and he knows you - even better if he likes you and sees you often.
If you're starting from scratch, be ready for lots of very direct questions.
good luck!

In person is best.
By phone if not.
Prepare what you like about her , no physical things probably though.
Tell him, he probably will understand and have mercy on you.
