depends on how many ppl you have working. If its a small place, one toilet is enough. If its more workers, I'd say add a toilet for every 3 woman or a urinal for every 3 men.

Depends on the number of employees and the size of your facility. If it's just an office with 20 employees then 1 toilet should be sufficient. If there are more I would suggest 2 and if you're spread out over 10's of thousands of square feet I would put more in just so employees don't have to spend 5 min walking just to get to the head.

Ok, first of all, all workplaces should be set up for "mixed gender" working. Secondly, it TOTALLY depends on the number of employees at a particular office.
