Find a situation where you can casually engage her in idle conversation. For example, happen to be at the soda machine when she is, and comment on something going on in school. If she seems warm and friendly, then wait for a lull in the conversation and ask her out. It should be something definite, like a movie on friday at 5 o'clock (call ahead to make sure of the times, prices, etc.. Women like men who are confident and sure, so having specific plans is MUCH better then saying "wanna go out sometime?" By beginning with idle chatting, you can transition easily into the invitation. And don't worry about rejection. If she says no, it's probably because she's nervous or already has plans or someone special - it doesn't mean you failed in any way. Just accept her answer graciously and move on, for example, "Okay, no problem. Have a good day!" By saying this confidently, you retain your dignity and build your own self-image. Good luck!

This is actually my post from a question earlier Titled "How do I talk to this girl" but I thought it might help you too...
Well first of all...once you do get the courage to talk to her make sure you are yourself...(otherwise being so nervous it will show... which she might also like)
Make sure you talk about her...what she likes to do, etc...something she can relate to. (this is also a way to get her to talk more so you don't have to do the talking until you're more comfortable.) AND REMEMBER No matter how dumb you think you sound...she has heard worse and as long as you are sincere and polite there's really no such thing as saying something dumb. (unlesss she's a @#@$@, which can also be fun, but thats an answer for another question...)
Also you can always get her email or her instant message and use that as training wheels,,,ya know chat a little and learn something about her before you really start talking.
Also remember she is just as nervous as you.
ANNNNNND...my last point...the first conversation or two are always the toughest, as you get to know her better the convesation will be easier.
I used to have the same problem...so when I really wanted to talk to girl I would think of three things I know about her and start off by asking about those things...then hopefully the convesation will be in full swing, before I ran out of my three talking points...
Remember girls are flattered to get asked out...so think of it like your just giving her a compliment...

however you want. Do you call her? Ask her for her number. Say something like 'maybe we can hang out sometime" and when you call her, find out what she's doing and if she's not busy ask if she wants to go somewhere. Don't just straight up ask her out, you've gotta hang out first. Then if things are going good you can say something like "so are you my girl or what?" (nicely of course)

All you need to do is forget you don't know how to ask her out. Just go up and tell her how you feel and ask her. The worst thing she can do is tell you no. Do it while you can. Well if you know that she likes you dude then why dont you find something that she likes to do and you also like to do and then one day come up to her and ask her if she would like to do that. Unless you're a good singer...dont sing to her lol
