You need to focus on the impact on the family. The smoker pretty much knows the impact on him/her. What they often don't think about is the impact on those around them whom they care about.
For instance:
A daughter getting married: "I wish dad was giving me away, but he refused to stop smoking."
A wife facing hard times: "I loved my husband, but I hate that he died so soon from smoking. It's been hard on us making ends meet."

draw a picture of a frog an put it next to you and on it say "please don't smoke I might croke" because as you know smoking not only hurts the person doing the smoking but the second hand smoke is bad on the person they are around.

The "smoking makes you ugly" campain showed on billboards what smokers look like after years of smoking. Apparently it was too effective, and was pulled.

You're not going to make that work. If they smoke, they already know the dangers and don't care. Not much that you do is going to suddenly change their mind.
