Since we don't know what his interests are it's kinda hard to make suggestions. If he smokes, a nice Zippo lighter might be nice. Get his name engraved on it for a personal touch. If he likes to read, a gift certificate from Barnes and Noble. Just go with his interests or hobbies and whatever you get, he'll like.

Take him home make him a mind blowing meal, listen to soft jazz sitting on the sofa with dim light, talk about life and love. And let him get to know how much you care for him.
I guess you are the best gift you can give to him.

If he usually treats you to dinner or lunch, why not invite him out for a nice candle-lit meal? This is a gift that is always appreciated, lasts for hours and is quite easy to find.
Do you know his favourite food?
Is it Indian, Chinese, Thai?
Reserve a table and have a birthday card ready to place in front of him.

Depends on your age of course and if you live at home or alone. I'd say something that wouldn't embarrass him, like a dvd you'd like to watch together.

Why don't you make him a compilation CD of his favourite tracks - then you don't look like you're trying too hard but it's a nice personal gift.

Get him the ole shot glass.. Ask him seductively to fill it with his 'man juice'. Once he has done the deed (and you are allowed to help), drink that stuff down like it was grape kool-aid.
