Over the years comedians have tried to out "sick" each other in their telling of the joke.

Tom's right - it's the Aristocrats. There was actually a film done a couple years ago about it.
What was funny was that Bob Saget, the guy who hosted America's Funniest Home Videos for the longest time and the guy who played the dad on the TV show Full House, told one of the raunchiest, most vulgar versions of the joke - really caught a lot of folks off guard.
