I would talk to her about it. Tell her you feel disrespected when she is late. But I think that leaving is disrespectful to her, unless you tell her that's what you plan to do. You need to come to an agreement that if she does not care to be on time, then you will leave.

I think going ahead without her might be acceptable once in awhile, when there's no major consequence. For example, if you are to go to an event that starts at a specific time, and you said to Mom, "Look, I am not willing to be late for this. If you are not ready in five minutes, we will have to go without you." If there's no huge disgrace in her missing it (like a relative's funeral or something), perhaps she'll get used to getting ready on time.

But play fair with her. It's clear she is not able to estimate how much time she needs to get ready. So start at like half an hour to warn her of the time, and then again at ten minutes, and at five and at two. Once she gets ready on time even once, praise her and tell her you hope she'll get used to being on time, because it is so much nicer for everyone.

For Mom, you need to wait as long as it takes. For anyone else 15 minutes.

Since it's your mom I'd have to say wait... but I'd hope that you have the kind of relationship where you can lie to her and tell her you need her there/ready a half hour before you really do so you'll all be ready on time... =D

Being late is about control. 15 minutes would be my max wait time. Mom or not. I'd be outta there!

You haven't said how old you are. I would imagine that your Mom has waited on you throughout, the years, maybe it's your turn to wait.

I leave. I don't make people wait for me so why should I wait for them.

Tell her nicely it bothers you and you only have 15 minutes to spare. Stick to it every time, although it may be hard at first, it will get easier as she gets the picture.
