If you don't live in the City or county where you were born (and this applies only in the United States) then you can contact the Clerk of Court in the City or county where you were born and they can give you the address to mail your check and request to. (There will be a fee)
Depending on the state that you live in, you may also be able to obtain a copy from the Bureau of Vital Records in the capital of the state.

It depends on the State. Kansas you have to write to the Division of Vital Statics at Topeka to get a copy. Our local counties do not keep them.
