This was probably started during the witch hunts "The Inquisitions". The jailed were tortured until the court heard the admittance of guilt. Then the accused were executed. Until then they were shown "No Mercy" and whipped and beaten again. Mercy being death and freedom from further judgment.

I have a favorite quote that goes:
Grace is getting what you didn't have coming, and Mercy is not getting what you did have coming.
With this in mind, I take "no mercy" as getting exactly what you had coming.

Well if u are looking for meaning it means for any reason(Sympathy, kindness or humanity) the punishment or the tough doing wont be eased.
