What are his hobbies/interests? Are you artistically talented? Do you write poetry? If you know what his interests are, then buy a gift that reflects that, for example, if he likes fishing, maybe a new rod and/or a reel, or a new tackle box. If you're artistic, ANYTHING made by you is going to thrill him. If you're a writer, put into words what he means to you. I can guarantee he'll treasure a gift like that! (I wrote a poem for my father's birthday when I was in high school a hundred or so years ago. He STILL has it, carefully folded in his 'treasure box'!)

The very best gift you could give is your time with him. Presents are nice, but he would rather have family close. Why don't you plan an evening with him, dinner, games, talking, sharing, anything. You may even learn something about him you didn't know.

If he is a sports fanatic, tickets to a football game, baseball game (next season), basketball game, or any sports game he would enjoy. Order them far in advance so you can go with him, and invite other members of the family so it can be a memorial event.
