you need to work on your own schedule, especially when going into rounds.
i'm doing 3rd year while taking the kaplan test prep for the usmle. I've built myself a solid routine to study for them both (and get good grades), go to class, hospital practices and go to the gym.
then again, I wake up at 5:30am.
set up a routine where your reading time per subject is balanced and coherent with what it requires. you probably already have noticed, the more you read the quicker you get so you'll start to find more and more spare time adding up.
listen- the best results you don't get by killing yourself reading through the night continuously. you need to study, doubtless, but in a balanced manner, with small 5-10min. breaks, and distributing things (don't study 10hrs the day before an exam but 2hrs/day the preceding 5 days). you get much better results by small quantities which consolidate properly during the time you're off the subject.
another thing is to determine what you portray as "fun". relaxing at midday? or going out at night? because that helps you also set up your schedule: if you find yourself slacking off for half an hour during lunchtime, take that time to read or do a lagged piece of work or whatever, instead of waiting to get home at the end of the day to do it. you wind up having a tighter packed actual _work_ schedule (say 7am thru 5pm hard work) but you'll earn quite a bit of time at night (if the case is such that you prefer having nighttime spare time).
oh and if you go out 1) don't get too drunk or you'll become useless in the morning and 2) don't drink and smoke (or don't smoke too much- ciggies I mean. no pot.)- hangover multiplies the headache bit by like a million every time you smoke 5+ cigs a night (personal experience).

In med school, sadly, the ratio of studies to fun is about 10 to 1. (Sneak in some extra fun when you can, and good luck!)

I'm sorry but this is the point in your life when you decide on a career, study has to come first. If you have spare time don't waste it on TV, sitting around moaning etc, go and do something spectacular.
