While I may not know of a website, I know of a great book. The Pagan Way of Living & Dying, by Wiccan Authoress; Starhawk.
Most Wiccans believe in one of two possible outcomes after death, if not a combination of both. While belief in reincarnation is not necessarily a key part of Wiccan, many Wiccans do believe in it. There is also the theory of the Summerland - the Wiccan equivelent of Heaven, and rather similar to the ancient Greek and Roman Elysian Fields.
Those who believe in both see it either as a dual possibility, or in rebirth after spending some time in the Summerlands.
A couple of chants I know that could apply are:
Hoof & Horn, Hoof & Horn
All that dies shall be reborn
Corn & Grain, Corn & Grain
All that falls shall rise again.
&
We all come from the Goddess
And to Her we shall return
Like a drop of rain
Flowing to the ocean.

Mostly it depends on the person's wishes. Some Traditions have rituals they use, but mostly its up to the family and the wishes of the deceased. As Pagans belive in re-incarnation, that you will return and that every death here, is a re-birth in the next life, we do not make as big a deal out of death and funerals as other religions.
I want everyone to party and celebrate my life when I die, and scatter my ashes in the Canadaian Rockies. Whatever else they do is up to them.

What type of Wiccan was s/he? A celtic tradtion would be different from a Dianatic trad, or a Norse Trad etc.

as with most things Wiccan and Pagan in general each grove or coven will have their own beliefs. it would be best to check with the individual...pre-mortem...on what theyr wishes are.
