Must I replace the stem, or is there a way to fix this?

JB weld or a self-tapping screw will NOT give you the clamping force necessary to keep a stem tight enough to ride with. Your only hope of salvaging the stem, if you ABSOLUTELY MUST keep it, is to drill out the hole and put a helicoil insert in it.

If you don't know what a helicoil is or can't do the work, then BUY A NEW STEM. Don't listen to anyone who says otherwise, unless you enjoy your loose stem throwing you to the ground.

Next time, grease the bolt threads and use a torque wrench.

You can get the hole rethreaded or go down to the hard ware store and get a self tapping bolt or screw, take the Stem with you.
