Contact your closest Canadian Embassy/Consulate. They should have application forms handy, or they could send them to you by mail.
You should fill them out, send in the passport photos and documents as per the directions, as well as the fee, and send it in to the address that they specify.

You should call the Canadian Consulate nearest you and they'll mail you all of the forms. If you have any questions, they have an operator on hand to answer any of your questions between 9-4 I believe. I'm a Canadian Citizen and US Resident as well and I've renewed my passport a couple of times with no problems at all. They process and mail passports rather swiftly. That's Canada for ya :)

I think you can just renew it. But they are doing new passports now that have an electronic chip in them, so you may have to reapply for one. Look at the governments website to make sure. =)
