It is a reference to how clowns have that painted on smile that is visible to everyone. It means that you are hiding what you are feeling under a painted on smile, pretending that you're okay when you really want to run away and cry somewhere.

The line was made famous in the late 50's in a song called "The Great Pretender" performed by The Platters. Listen to the song it twill explain it all...

it's wearing my heart like a crown (not clown) to rhyme with the word clown in "the great pretender" by the platters.

i think that it means the person is so head over heals in love that everyone can see it. He feels that he looks looks like a clown by being so overt about his feelings, but he doesnt really care because the person he is courting is worth looking silly for
