For baked goods(cakes, muffins) and pizza, it REALLY makes a difference. For casseroles, and baking meats, etc. it doesn't matter too much, so I don't always do it.

Yes, always ! All the recipes are tested in an oven that is already at the temperature required.
When you think that it can take up to 10-15 minutes to get to 400 degrees, it will make a difference if you put a cake or a casserole in a cold oven and start timing it then.....

If the recipe calls for pre-heating I do.
Do you understand why one pre-heats an oven? Usually when pre-heating an oven gets hotter than you may want it to be initially with certain foods or recipes. A pre-heated oven will give you a constant heat and therefore more even cooking so you will have less chance of undercooking or overcooking.
If the recipe does not indicate the necessity of pre-heating I don't bother.

I should, but I don't always.
If I am making a complex recipe, yes, since I am in the kitchen prepping.
If I am making something to eat now (like fish or chicken), it goes in the oven as soon as the oven comes on.

Yes, that way the oven is at the right temperature when I cook. Otherwise your cooking time will be thrown off, and it becomes necessary to constanly open the oven to check your meal for doneness, and that makes the heat unstable, too.

Sure I preheat my oven. If it is not hot when you put the cake, etc in, it will not bake right and you will have a mess.
