There is no monthly fee with most credit cards although there may be an annual fee. American Express for example.
If your credit score is pretty bad there are some cards that do charge an monthly fee. The chanrges should be shown on your monthly statement.

You may pay an annual fee like the VISA I got through First Premier Bank to rebuild my credit(I wouldnt recommend them for your long term credit needs) and the other 3 credit cards I have that I am not using all are NO annual fee cards and one of them is American Express and this one along with my Phi Theta Kappa Mastercard was issued through MBNA(now officially a part of Bank of America). If anything you're better off with only 1-2 cards because folks are putting themselves in the poor house with more than 3 credit cards with high interest and credit lines.

If you have a zero balance, then the only time you would pay any fees is if your credit card or charge card has an annual fee. I suppose sometimes it could be broken into monthly payments, but usually it's a one time annual fee.

Many credit cards have no fee at all if they are paid in full each month. Other credit cards charge annual fees. Most revolving credit cards charge interest on the average balance, which means paying a higher than the published rate. Credit cards are dangerous and should not be used if they can’t be paid off in a short period of time.

No unless you have some left over balance. I haven't been using any of my credit cards, but there is a balance due, so I still have to pay the minimum due whatever they are asking for a little each time. For example if one of my creditors want $28 each month, I give them $30 or $35 depending on my budget until the balance I owe is $0. that's when they will stop mailing me those dumb bills.

Well if you have a balance yeah you pay a monthly fee, that's how they make their money off monthly fees and apr's. If your balance is zero then you don't have to pay a monthly fee

Nope....at least not in general. At least not so long as you don't carry a balance.
Many subprime credit cards charge annual fees for having a card.

As long as the credit card is open you get monthly charges. The only thing you won't get are finance charges, because you haven't charged anything.

Check your agreement. They are all different. They will sap every dime they can from your pocket. The lower your credit score the worse they are on you.
