Maybe say, 'I have a dentist appointment, wanna come? We can hang out later after it, so you could just listen or wait for me or whatever." Stay casual so she doesn't get offended.
How does she need to brush her teeth? Nasty.

You sould be honest with her. Talk to her privately about the issue and don't insult her. The best thing to tell her is that you want to talk about a personal issue that's been bugging you and ask her to try not to take offense to what you tell her.

eewh! that's gross. i couldn't date someone that has smelly breath and plaque on their teeth all the time. maybe take her on an overnite trip (if you're old enough) and bring the toothbrush for her... she should get the hint. if not by some damn good mints!
