"Formally disciplined" means that something you did (or allegedly did) has been noticed and recorded by the employer, and that you have been notified of this, all in accordance with the company's disciplinary policy.
For instance, if you made a mistake (or your employer believes you've made a mistake) and someone informally chats with you about it (or informally yells at you about it), this is NOT being formally disciplined. However, if there is a record of the discussion and it was in keeping with the company's disciplinary policy, you've been formally disciplined.

the key word is formally disciplined this often means somthing that has been recorded on paper and in your file, so that if your boss wants to can you he could say look at this, this, and this

it is one of the initial steps for justifying your termination. They can't just fire you without a reason. Formal discipline puts you on notice to improve or you will be fired.
