Federal Aviation Regulation 129.29b states "no person may smoke and no operator may permit smoking anywhere on the aircraft (including the passenger cabin and the flight deck) during scheduled passenger foreign air transportation or during any scheduled passenger interstate or intrastate air transportation." That makes it illegal for an airline to allow smoking on any commercial flight within, or to or from the United States.

There are probably a handful of overseas airlines that still permit smoking. However, I suspect their aircraft are probably stuck in the '80s along with their policies, and I would not recommend flying with them.

This is based on old research I did (about five years ago) so may have changed.
Aeroflot have a very limited number of smoking seats.
All flights to/from Japan are required to have smoking seats (under Japanese aviation law).
Deep Vein Thrombosis is not caused by cramped conditins, it's caused by bad air-quality. Without smokers, they run the air-conditioning less frequently and on a lower level. Smoking bans on planes were adopted by the airlines because the decreased aircon meant they used less fuel (essentially Kerosine) NOT because they give a dingoes kidneys about your health..
All airlines' fleets are now less safe. Invisible hairline cracks in the fuselage used to be first noticed by the nicotine stains that formed around them as the cabin atmostphere leeched out. No smoking = no early warning safety system.

None at the moment. But I've heard that one or other airline is starting a smoking flight.

Only foreign carriers. No U.S. airlines permit it.

I about choked to death on a Turkish Airlines flight a few years ago. The air was blue. Yuck!

As far as i am aware only on domestic flights in Egypt. Most country's have signed up to different conventions such as Warsaw or Montreal and is now illegal to permit smoking when flying to or from these countries.

I think you're not allowed on board the airline
Through all my travels none of them allowed smoking on board the plain
it will cause smoke you know

Oh for GOD SAKES give it up. smoking is bad for you period. is much worse on an airline.
