Mia only - no article. Same ground rules apply throughout the Romance languages in general, I believe.

You need to use the article la. So, it's la mia famiglia. For example, my family lives in Roma= La mia famiglia vive in Roma.

of course...in this case doesn't mean a possession but that you belong to "your" family...i don't know how to give you a better explanation, but for example if you say "may family is composed by 4 people" in italian would be "la mia famiglia è composta da 4 persone". "mia famiglia" wouldn't be correct in any case.

My italian is only thin (26 years ago I stopped learning), but I do remember anything with possessive pronome needs the article in front.
Examples:
il suo nome (his name)
la sua nome (her name)
la mia famiglia (my family)
But I am german, not italian, so I hope a real (native) italian speaker will either verify this, or correct me.
