If you were injured on the job you should file for Workers' Compensation. State Disability Insurance (SDI) is for illesses and injuries that happened outside of work. You can file for Workers' Compensation by submitting a Workers' Compensation Claim form to your employer by mail or in person. Here is a link to instructions and the form:

http://www.dir.ca.gov/DWC/iwguides/IWGuide01.pdf

If you cannot open this link, ask your employer for this form.

It is illegal for an employer to punish or fire you for applying for Workers' Compensation.

If you're having difficulty getting information about your claim or the claim form, call the Division of Workers' Compensation at 800-736-7401. They can answer questions about your specific situation and advise about whether or not you should apply for SDI.

If this injury and your inability to work is likely to last more than 12 months, you should also submit an application for disability benefits to the Social Security Administration as soon as possible. Call 800-772-1213.

Good luck!

You may be entitled to disability if you are disabled due to your injury. Contact your local Social Security Admin. office and inquire. They can send you forms or you can go to their website and download the application.

Yes. Ask your doc for a referral to state disability.
