Write that person a letter expressing in a mature way how you have appreciated their wisdom and knowledge over the years. List some of the things they have taught you and how you've applied them.
For example, your first grade teacher taught you to read and write, and now you're a copy editor or a reporter. Or, the 8th grade math teacher who spent extra time with you in pre-algebra, and now you're a civil engineer, using higher math all the time. Or, your piano teacher... and now you're about to give your senior trombone college recital. You get the idea.

Then, thank the person again, and enclose a $25-$50 gift certificate for a nice local restaurant or a similar gift card for an appropriate store. Even if you don't enclose a gift, just receiving a thank you letter will be a treasure that person will hoard for his/her entire life. You will have no idea how much a letter like that will mean to your mentor!!

I'm assuming that you've treated them like dirt, mocked them, and been a general pain in the *** for quite a while before seeing the error of your ways.

In classic form you would straighten up & fly right (another good song) & become a productive member of society.

Beyond that I think naming your first born after them, and perhaps a bottle of wine some flowers would do the trick nicely.

you are very thoughtful....
give them something which reflects your entire experience with that person. It could be a simple a song or, well, I can't imagine your ultimate feeling towards that one person. GOOD LUCK!
