Gas is caused by a number of things, but usually it's from the food basically fermenting in your gut (courtesy of E. coli bacteria, among others). Veggies have lots of nitrocellulose, which we don't digest very well, which is why it tends to ferment and cause gas. Very spicy foods can also cause gas by irritiating the stomach lining and causing it to secrete more acid. Very cold food may be irritating to some people's stomachs and give them cramps, but it is just water, and water has nothing in it that makes gas bubbles.
Your customer is just making up an excuse for avoiding ice. Perhaps she has sensitive teeth or something else. Or, she could be 'just crazy.' Just give her water with no ice and shrug your shoulders.
You can laugh at her if she orders ice cream, which contains small ice crystals, and therefore should give her gas.

Place the ice cube on a heat sink. This is a piece of metal (usually aluminum) that has fins on it. this will carry off the heat, or in your case, cold. They are readily available at most Radio Shack stores, just ask for a piece of heat sink material. The larger the heat sink, the faster the ice will melt. I use just such a thing to thaw out meat to prepare for cooking. It takes much less time to thaw, than just sitting it out. This uses both principals of convection, and conduction. Convection since the fins at a lower temperature will naturally force more warm air through them, and conduction since the ice is in direct contact with the surface of the heat sink. This requires no use of electricity, gas, or any other form of artificially generated heat. Let me know what you think.

Yes they can after u have certain surgeries because sometimes our bowels can slow down and back up and then ice chips do something to that and it makes u gassy. There is a more technical way to explain this but its true, after my hysterectomy my doctor told me the reason but I can't remember the terminology he used.
