We think that this prayer was written by Dr. Rheinhold Niebuhr, a theologian, in his book "Practical Christianity", but even Dr. Neibuhr casts some doubt on this.

There is also evidence that this prayer came from early Christianity, as we have a reference from a Roman philosopher, Boethius, in the 5th century.

Still another reference comes from a "General Prayer" published in England in the 14th century...whatever the source, it did not come from the Bible but was almost assuredly written by Christians.

Most likely that prayer in question came from a person that was incarcerated at that time. If you do your research most all of these 12 step programs are works of inmates developing a program to get themselves and others back into a normal lifestyle whether it be alcohol anonymous or narcotics anonymous these programs were thought out while being in confinement and serious sessions of soul searching

It is not in the bible.
I was in AA, and that is a long prayer.
I believe that it can be found in a copy of the book Alcoholics anonymous.
