I would just let him go, the same thing happened to me, we got back together, 2 terrible years later we broke up again.

Tell him your honest feelings about what happened and the mistake you made by taking him for granted in the past. If he does not accept what you have to say - don't get mad at him because no one likes being taken for granted. If he takes you back - DO NOT make the same mistake again or you will really mess with his head and emotions for the next relationship he is in.

Hind sight's 20/20,you should have realized what you had when you had it.And honestly what you did must have been pretty bad,to make someone who loves you leave.
You can't make a person do what they don't want.But you can try to show them you have changed,and mean really changed.Are you willing to put the work in to do that?Or is it not that important to you.
If not cut your losses and walk away,for his sake and yours.
