Many are born without a conscience . Some , I believe were exposed to ruthlessness at an early age . Others are just downright opportunistic and have no capacity to love anyone but themselves-which is odd in itself because they have an embedded self-loathing .

Because other people let them, especially parents. For example, small children that do stupid things; if every time a kid does something, as in making an ugly scribbled drawing and then telling you its a picture of you you respond by saying "it's so nice, very good son!", then not only will you be narcissizing him, you'll be turning him into another cog in the machine of mediocrity; your response, if you are a good parent, should be "Nice try, son, that's just a nasty scribble, don't look like no one I ever sawed. In a few more years you might perhaps draw something decent the whole fambly can be proud of". That should get the kid to really wanna draw well..... or maybe just push him off into the delirious world of psychosis, but that will be one unnarsissistic schizophrenic.

Narcissism is considered to be a personality disorder and I am not sure how they get that way but those with the most severe cases of it need to seek treatment and to find a way to look beyond themselves and their own needs...volunteer, do things for other people...

As a professional astrologer, I've noticed that certain Sun Signs are narcissictic. Capricorn is the worst.
