typically your license back says if you want to be a donor, just sign it up, if you are under age then you probably need to talk to your parents, noble thought, good luck

Like it was said earlier you can sign the organ donation portion of your driver's license but not everyone has a license.
You can register at Organ Life as a donor and you will get an organ donor's card.

You can download and print the card for free. Just use card stock in your printer. Or you can order the two-sided card from them

You may obtain an Organ Donor Card from the Public Information Desk of any Hospital. You will need to get at least three. Place one with your Will, Carry one in your wallet. Give one to your family Doctor.

When you renew your drivers license, they have an option for you to become a donor, which is where it is listed. You don't normally get a seperate card unless you get it done elsewhere.

I think that you can have it recorded on your drivers licence. In DE when you renew your drivers licence you are asked if you wish to be an organ donor. If you answer yes, it is recorded on your licence. davidcaloy@yahoo.com
