2. If the doc can help me and then wants to sing a dark hallelujah to a fictional entity, I can't see anything wrong with that. It's no different in pragmatic reality than thanking God or Winnie the Pooh.

Prayer!
And the first thing I would pray for is a Doctor that isn't being ruled by Satan!
Guess what? God would so EASILY take care of that prayer. I know I would have a new Dr. Lickity-Split!
Then... I would continue with prayer AND enjoy the blessings from my new Doctor!

I would rather die than to receive treatment from a professed Satanist.

Good question. I would choose prayer. I believe in going to the doctor but given those choices, I would rather have prayer. Prayer has helped me more than medicine and I have had to have both.

I'd pick #2. I have no problems with Satanists. Fun bunch of people. I'd actually be quite happy to have a Satanist doctor working on me; since their basic goals are fulfillment of the self, he's going to do a better job than most in order to keep his money and prestige.

It would be irresponsible not to make use of all means that God has provided, and which therefore are not opposed to His divine will. The third option could not be used because superstition is directly opposed to the will of God. However, prayer and medical intervention are legitimate sources of help which are not opposed to the will of God. The personal religious convictions of the doctor are not relevant. You can accept his help, and thank God for his skill. Even if he doesn't recognize God as the source of all he has, you can still recognize that fact and thank God for it.

If those are the only three choices I have, I will have to choose #1. God does work through doctors, but I doubt there are any doctors that worship satan, they may be agnostic or even atheist, but never satanists, they do not believe in healing.

Edit: If prayer worked, why would God need you to go to a hospital to be healed? Prayer does nothing, Christians, and you're ******* stupid for thinking that it does in spite of evidence.
