One of the guests we are inviting to our wedding is both a Ph.D. and a reverend. Is it proper to send the invitation to "The Reverend Dr. and Mrs. John Smith" or does one title outrank the other?

The Reverend Dr. is proper etiquette, you are correct. Even though a title may outrank those are both prestigious titles and you mention both.

If you're close enough to invite him to your wedding, I would just drop all titles, and address the invitation to "Mr. and Mrs. John Smith". I don't think he'll mind it.

My brother in law is the same - he's a priest and has his doctorate, so in his case he's Rt. Rev. Dr. Joe Cool. In the case you give, you are correct.

I believe that it is the proper way, since he has a degree in both, she is aware of that so I am sure it is alright.
