Here is the link to Dragon Naturally Speaking - I have used it before and it is very accurate after you train it to recognize your voice. It costs about $200

Another option is to check with your ISP - At one time I had AOL and AOL offered Speech to Text for free. I'm not sure if they do anymore or not. Other ISPs may or may not offer it.

if you can get your hands on a beta copy of vista ,this has a speech to text editor built into the operating system,and also a text to speech

To be honest,you really can't do better than what is pre-installed with windows. works great.
Click start-programs-accessories-accessibilty-narrator.
Job done.
