Technically, according to the Bible, a heathen is anyone who does not believe in God.

The Philistines were the main "heathens' in the Bible, as were the Egyptians, the Assyrians, the Babylonians/Chaldeans, some of the Greeks. Today, I think the "heathens" are primarily in Asia (except for South Korea), the Indian Subcontinent, Africa, and the Middle East.

Anyone who broke God's law they termed as heathens.
They had laws against lieing, stealing, murder, adultry, fornication, astrology, gambling, drunkenness, even greed.
The nations around them practice human sacrifice, had no moral laws and had temple prostitutes where they used young children in very immoral ways. They were violent and tortured people and were cruel to animals. All things forbidden under the law.
