Basically it allows you 12 weeks per year that you can take time off to be with family members or yourself for extended mecidal conditions and not lose your job.

The first two are right, but you won't get any benefits while on the leave. It's a thing they give you to allow you time off for medical purposes, family or yourself, but they won't pay you for it, they will hold your job for you and that's all.
And you can't collect Unemployment because it's a voluntary out of work situation.

Ben Dover is correct, but its only for companies that have a certain amount of employees - larger companies.
