That all depends on the printing, the number of notes, if it contains a concordance or index, the comments and references, and many other factors that are the same as any other bound codex (book). Why do you ask?

Depends on which Bible you are using. If you are referring to the books of the Bible that is different.

That would depend on what version of the Bible, whether, KJ, NIV, Amplified, etc. the print used ( large print, and the like ).
