when you take your dog to the vet and it is your first time taking your puppy do you have to fill out any paperwork if so can you explain

Of course. They want your name,number,address, and the animal info...species,breed,color,sex,age. That is the basic information. Many ask other questions, others ask no more than this. IF you are planning on paying by check, they may ask for drivers license number and SS#, and maybe employment information.

With our first puppy, we filled out a small questionairre and a registration sheet (we have to by law here). The questionairre consisted of asking where I had obtained the puppy, if I know any history of the parents, how the puppy will be living (mostly outside, mostly inside, etc), and what kind of food I feed it.

Usually either a vet tech or a receptionsist will ask you questions concerning your name, mailing address, phone number, pets name, age/birthdate,breed/mix, color,if it had any vaccinations already, if so when and what type (be prepared to show proof), if it was dewormed with what and when. After taht the tech or vet will ask you about stool, appetite,activity, etc.
