While there are definitely some people who delight in hurting others, or who act selfishly on purpose despite knowing how they are hurting others in the process, I think most of the people who "do evil" actually think they are doing good.
People leave marriages because they feel unhappy and think they are doing everyone a favor.
People promote social or political causes, imposing their ideas on others without really consider the other person's feelings.
People in some countries imprison or kill their own citizens for the "great good of society."
People who do evil while thinking they are doing good are the most dangerous people to deal with, because they have passion in their cause and because they believe that the end justifies the means... and they are incapable of changing in themselves what they don't realize needs changing.

Actions speak for themselves. Everyone has a God given conscience, and will be judged according to that & land laws, etc...
A persons conscience would tell them that to blow self up in a crowded restaurant or a grade school for some so called religious or self-righteous purposes is evil. But some people have their conscience seared with a hot iron. It is still an evil. The process that caused the conscienced to be seared, and the evil act that follows, God will judge.
Also, all sin is evil, so following an evil religion is sinful & evil, even if deceived in it & believing a lie.

Well that depends on your defenition of good and evil. This has a lot to do with what is socialy acceptable in that particular part of the world. Some socialy acceptable behaviors are not so in different places.

Intention vs. action, eh? I'll take a stab at it.
Though I'm not a Buddhist myself, I tend to follow the Buddhist line of reasonings on such things; and if your intentions are pure and virtuous, this can often override the negative results of such actions. It's the same question as, if you give a homeless man money, and he buys drugs with it, was this bad or good? Your action was good, and your intention was good. How other people take it is up to them.

we are all evil, it's only through Jesus that we are made ok to approach God, sorry but it's his rules not ours, I wish I could say I met God when I was doing loads of charity work, or in church pretending to be a nice person, but it happened on the beach when I was a flirty tart with depression, I only hope you meet Jesus before you die, pray you do.
