If you have to come back to the hospital they can refuse to treat you and your insurance can refuse to pay. The usually fill out a form called an AMA (against medical advice) that prevents them from being liable for your actions.

If you're a 'competent' adult, then nothing is likely to happen. No one can make you stay in hospital/receive treatment.

If you are deemed not competent - ie. not mentally able to make the decision about your health, then if it is felt you require further treatment, the police can be asked to return you to the ward.

If you want to leave early, it is polite to inform the ward staff, and sign a form to say you are leaving AMO - against medical advice. Saves staff the time needed to search for you.
