Examples of real life software crisis?

I'm doing a short report on software engineering, I know what a software crisi is but I cannot find any real-life examples.

Can you point me in the right direction or suggest a good site for me, as I could only find wikipedia when I typed in the search engine. Thanks.

use the millemeum bug because that was a huge crisis the tills messing up etc
