Too much lubrication (some condoms are already lubricated and the woman can get too wet), vigorous movement, loss of erection, change of position, sudden clenching of the vaginal muscles, cheap condom, condom that's too big
Never rely on condoms to completely protect you from pregnancy and STDs- use a spermicide or the pill AND condoms, and do not hit the sheets if you are not sure that your partner is clean.

Everybodys different in Size and Fit!(And Condom makers don't have it all figured out!!)
You also have to consider if you are making Love erratically or gently-The Obvious outcome of erratic Loving is for the Condom to slip off!
You could Tape it on? Ouch! Lol

Well, either a) the guy is using a condom too big for his business or b) he stays in too long, and gets kind of soft before pulling out

It happened to me the first time I used one. I was so ashamed until I read the directions. Man I had it all screwed up. Yep, gotta put 'em on correctly or they can slip right off.

this happened to me before (and don't worry, i'm not small. to give you an idea, I cant even fit my pekker into a toilet paper tube) It is like a combination of him not rolling it down all the way, coupled with friction of in/out grabbing the condom and pulling it off with each thrust.
Nothing spells buzz kill like your female partner asking you to reach inside her and pull out a condom that fell off... just gotta laugh and dive back in :)

Never heard of such a thing ! It's usually the opposite,(to tight) I have a very hard time getting them off

yeah that's never happened to me and i've wondered that question myself... maybe they just start losing the erection and they really don't know it until it's too late... or they just have a small "peck" as you would say haha.

I've never had that problem. Until recently when things got alil ruff !
and the dang thing came off. luckily i cought it b4 it was too late.
