It could mean a lot of things. It could mean you said something stupid, or you have something on your face, she could be thinking of how to ask you a question, or if she should ask you a question, or a million other things. Best thing to do is look into her eyes and ask her what's on her mind. If she says "nothing" its probably something but she doesn't want to say for whatever reason. Its up to you if you want to let this slide or not.
If she does tell you what's on her mind, remember that you asked; and go from there.

Depends on the stare - a "glare" is no good, but that classic stare with the glassy look in the eye and the pursed lips means she digs you. Either that or your fly is open.
