Only if you have over a staff of 5 I believe.
By law you have to provide an eating area, a microwave, tables and chairs, a sink and water.

I believe the answer to this question would be a company related specific question. these can vary from company to company i believe. the only way to find out your answer for sure is to ask your questions to someones within the company or the HR department

Yes keep them clean and do not take anybody else's lunch out of the refrigerator .
