I was told a long time ago that this phrase came from the practice of a jousting knight wearing his lady love's ribbons tied to his arm for all to see...hence wearing his heart on his sleeve.

Are you asking what this is a reference to? If so it refers to the fact that you can tell exactly what you are thinking about someone else as it shows on your face body language etc. you are easily hurt by others, you put too much stock in what others say and do. people who usually are this way are pretty self-absorbed. and need constant attention and confirmation from others, of their worthiness and love for them.

probably related to a english/french old velentine Eve practise of drawing a girls name from a box the male agreed to protect the girl for a year he wore her name in that year on his sleeve
