We went and had no problem at all, you queue up and pay at the front. If you go peak season the lift queue will be large, but if you fancy feeling really pleased with your self use the stairs for the first two levels, a fraction of the price, a good work out and you will feel you have accomplished something at the end of it.

As a frequent visitor to Paris I have found that the best time to avoid the queues is early evening when everybody has gone for their evening meal, and unless you are feeling energetic or if you are exceptionally mean, I shouldn't bother with walking to the first level, you can get the elevator from ground level to the top for only 12 euros. For an equally impressive, if not better view of Paris (as you get to see the Eifel Tower aswell) the Montparnasse Tower.

You can take the stairs to the second level at a reduced rate. To reach the very top, you must take the elevator.
The lines do get pretty long...arrive bright and early or later in the evening and the wait isn't that long. I would suggest hoofing it to the second level (which also offers the best view) rather than waiting for the elevator.

If you go out of peak times the queues are not so bad. We went on a week day in January and had no problems at all. If you see a million coaches parked outside, that is probably not the best time.
Having walked up & down to the second level (cos our teachers collected the wrong amount of money so we had to walk) - I would get the escalator everytime!!

http://paris.c-lafrance.com/LoisirsVisites/fiche_26478.html gives you some ideas about visiting the tower.
To avoid the crowd, go at opening or one hour before closing time. Less crowd also is climbing with the stairs and not the elevators, but you have to be in a good form and not dizzy !

I went in Spring (May I think). Not a problem at all. Maybe weekends and peak tourist seasons are busier but if you avoid those you should be fine.
