The definition of family is anyone related to you by blood or marraige.

This aunt would indeed be her great aunt, regardless how many times she sees or speaks with her.

Your question is a little confusing, first you refer to your stepmother, then your wife, then her mom's aunt, or is it her stepmom's aunt? If you're related by blood, then you're related, whether you speak or not. The relatives of a stepparent aren't technically blood, but may be treated that way for practical purposes. Of course, if someone falls in love with the blood relative of a stepparent, marriage would be okay, because they're not really related. Of course if you've been calling your intended "Uncle Steve" for years it's going to seem really creepy to everyone, and maybe it is... :-)

Your wife is. Her step-mother's sister is her aunt. And we all have family members which we can't stand, or do not visit, but they are still a part of our family..right?

Same rule applies here.
